Making a Bequest to MOM
You can influence the future of MOM! It is comforting to know that the good done in your life can be continued by remembering MOM in your will.
A bequest through your will is one way to influence the future of the programs and services provided by MOM. And, as a result, to affect the quality of the lives of many hundreds of people. MOM encourages you to plan carefully as you draft and execute your will. Wills should always be prepared with a legal advisor. If there is a program or service of MOM's that is of particular interest to you, you may wish to make a bequest in that area.
Below is wording for a will naming MOM. Please use this as you work with your legal advisor.
I give and bequeath to Middleton Outreach Ministry, of 7432 Hubbard Avenue, Middleton, Wisconsin, a not-for-profit corporation, the sum of $_________________ (and/or the securities or other property described herein, namely _________________) to be used as directed by the Board of Directors of Middleton Outreach Ministry.
Middleton Outreach Ministry is a 501(c)(3) organization: Federal EIN # 39-1484945.
